How to cover up cancer zodiac (69) tattoo?

<< A question for the male gender. Do you think tattoos are unattractive on the opposite sex?
My older sister got a tattoo that was meant to be a cancer zodiac sign on her wrist but people mistake it for a 69. She wants a new tattoo, either a different version of the cancer sign or just something that can be built off it, over the existing tattoo. Here is the tattoo picture:

Of course let manly.. But you know get the point.. Maybe she could incorporate it into a design that she likes somehow..
